So, Zaeed/Garrus for team leader, Samara for biotic specialist, does anyone have a good pick for tech specialist?? Everyone I pick dies, legion died, tali (obvious choice) died 4 DIFFERENT PLAYTHROUGHS, and fuck Jacob just died, I hear mordin will die unless you have him as the escort, so who is the best choice for the pipes?
When you talk to zaeed on the Normandy, he will tell you some war stories about how he alone survived sucicidal missions..... klick on that turian ship for example. He is hard to kill, but he is no good leader.
Only Miranda, Jacob and Garrus are qualified enough to be good leaders. Tech experts are Tali, Legion and Kasumi, if available. In the Pipes, both the Expert and the Fireteam Leader have to be loyal. Otherwise.....
On the long walk, only Samara/Morinrh and Jack are poweful nough to shield everyone until the end. All other biotics will result in loosing a squadmate.
The diversion team leader has to be Miranda, loyal Jacob or loyal Garrus, or there will be casualties.
Miranda is quite resilient as she can only die if you take her beeing unloyal to the final fight, or unloyal at holding the line and even then, there are many more going to die until her.
She can't die before the final fight, no matter her loyalty..... in the long run 4 example if no strong biotic makes the bubble and miranda is in your squad, it will always be the other squadmember, who is going to be raken. And you can't pick her as tech expert or escort for the Crew.
A loyal Miranda is the only Character that simply cannot die.
Yeah I just found out that Zaeed wasn't good for the team leader (first play through with him) I figured a good indication was when Miranda said "well at least he's capable" and as former leader of the blue sons who really only got kicked cause he wasn't a beurocrat i figured he'd be better then Garrus who lost a whole team on Omega. But even with Garrus Tali always died in the tubes
So.... Garrus and Tali are both loyal, right?
And she dies in the Tubes?
Or is she headshotted by Harbingers warp projectile in the cutscene (Jamming doors....)?
The latter is not possible, if both are loyal. You picked the right people for the job.
If she gets cooked inside the tubes, then you might have missed a vent control panel or were too slow.
Whats your class again? Remember you can skip the fights in the last room entirely by rushing towarda the last panel. Charge, Cloak, Adrenalin Rush, Dominate Drone work wonders here.
Infiltrator, when Jacob died that was my first play through as a vanguard, and yes she gets headshotted when helping close the door
that can only mean..... your squad leader or Tali is not Loyal, or your game file is broken and ME2 doesn't recognize the correct loyalty status of your crew which.... would be the first time I'd encounter something like this.

If you did everything the way it is explained, no one should and afaik from several 'no one left behind' playthroughs, no one will die on the mission.
For a tech specialist you'd use either Tali, Legion, or Kasumi ( if you have her)
tech,Garus as leader,Jack as biotic,and Grunt for excourt
Any loyal can do the crew escort and for the sake of holding the line..... it should be a weak char like Tali, Mordin, Kasumi or Jack, as they have a defence power of 1 each, while Grunt, Zaeed and Garrus have 4 each.
Assuming that you have no DLC then it's much easier to lose squad members during Suicide Mission (team strength overall is much lower without Zaeed and Kasumi). With all DLCs that's border line impossibility unless you do that on purpose (take not loyal squad members and assign them to wrong tasks).
From personal experience, when played vanilla ME2 first time (without importing anything) I've lost 3 people Grunt (loyal), Miranda (not loyal) and Thane (loyal).
With all DLCs I've never managed to lose a squad member.
Legion (loyal) in the pipe (although you have to be quick with valves and Ignore for the most part defenders); Garrus #1 (loyal); Samara biotic shield (loyal); Jack (loyal - escort for the crew); Garrus #2. My end-game team is always Samara+Mordin (loyal). That way I can keep eyes on good Doctor paper-thin Salarian health and prevent him from dying (+ Incinerate is an amazing power to spam and Samara's Throw/Pull quickly disposes un-shielded troopers dumping them off the platforms).
And finally. Seriously I did many stupid things with 'Loyal' Miranda (I really hate her - Jack said it well...) and simply put that Cerberus Nazi is just indestructible. Period. Probably would also survive implosion of the Milky Way Galaxy itself no problems. :P
Actually it's hard to lose people without the DLC too unless you do so on purpose, or simply haven't been paying attention/had a gross oversight like not doing somebody's LM or forgetting a ship upgrade. Sure the DLC makes it even easier for obvious reasons, but regardless of whether or not you have the DLC it's all about getting everyone's loyalty, all the ship combat upgrades, and sending the right people for the right jobs. If you do that it'll all be just fine. I generally send Mordin on escort duty because he's the first one to die holding the door if someone is going to die. That implies he's the weakest for that job, so I get him outta there. That said, I have left him at the door before and he lived. Keep in mind I do not have any of the DLC, but I still only lost people on the SM when doing so on purpose just to see how that goes when you lose people and import to ME3. Simply put, it's easy to make it out with everyone alive without the DLC too. Although the OP clearly has the DLC anyway as he mentioned Zaeed as a team leader. That's probably what's getting Legion/Tali killed each time.
The thing to remember, and this also addresses your issue of not being able to kill Miri, is that sending the wrong people for the various jobs generally gets OTHER people killed and not that character. So when you send Miri to do "stupid things" she will get other people killed, not herself. You will find it to be the same with anyone else in that regard. It's just like the Zaeed/Tali example above where sending a poor team leader(Zaeed) gets your hacker(Tali/Legion) killed. If you send a poor hacker into the vents, the team leader generally gets killed because they can't open the door fast enough, and so on. However, Miri does indeed seem to defy many conventions that hold for the rest of the party. The only for sure way I know of to kill Miri is to leave her disloyal and bring her with you to fight the reaper. Any disloyal people with Shep at the very end will die. My go to setup for everyone to live, with all loyalties and ship upgrades acquired of course, is Garrus for both team leader options, Legion for the hacker in the tunnels, Samara/Morinth on biotic bubble duty, and Mordin on escort. For my team at the end I always mix it up, but I generally make sure to leave Grunt and Garrus to hold the door. I have never lost anyone using that setup, and that's with no DLC involved. I have also taken either Grunt or Garrus before, and everyone still made it, but leaving those 2 at the door and sending Mordin on escort will 100% ensure a high enough door score for everyone to live if you have all of their loyalties.
It's an amusing irony that it's actually MUCH easier to get everyone out of the SM alive than it is to get them all killed.
