Here are the choices I made:
I only gained War Assets from the main missions and had only 2300 total by the end of the game.
I let Padok cure the Genophage but Eve died leaving Wreav solely in charge of the Krogan.
Kai Leng killed the Salarian Councillor because both Thane and Kirrahe were dead.
I didn’t see Kaidan in the hospital so I killed him when he sided with Udina and then I killed Udina.
I let Samara kill herself and then killed Falere.
I let Admiral Koris sacrifice himself.
I let the Geth VI upload the Reaper code, dooming the Quarians. Tali then committed suicide.
I got Miranda killed on Sanctuary.
The fleets were torn apart during the opening stages of the battle.
Cortez died when his shuttle was shot down.
Garrus and Liara were both killed on the approach to the beam and with their deaths, all of my original crew from ME1 was now dead.
I couldn’t convince the Illusive Man to stand down and I got Anderson killed.
Once I reached the Catalyst, he explained that due to the damage done to the crucible, I had only one option to defeat the reaper threat: Destroy.
And so I did. The Citadel, the Mass Relays, the Geth, EDI, even the Normandy. I destroyed it all. Nothing was left standing by the end except for debris.
And so ended the story of Commander Shepard. His legacy would be a Galaxy left in turmoil and ruin with an uncertain future ahead of it.

Here are some of the decisions I made:
I did no Loyalty missions except for Garrus’, Tali’s,
Thane, and Samara’s.
I let Garrus kill Sidonis before he got the chance to explain himself.
I got Tali exiled from the fleet because I didn’t handover evidence from the Alarei.
I let Kolyat kill his target and ruined Thane’s life.
I alerted Morinth and she got away.
I did 5 side missions before I did the Suicide Mission and got the crew killed.
I did none of the Normandy upgrades.
Jack, Legion and Thane died on the approach to the Collector Base.
I sent Jacob into the vents and he got the Garrus treatment of a rocket to the face.
Grunt got eaten by the seeker swarms and Zaeed got killed holding the doors.
Chakwas also died because I sent no one to escort her.
Mordin and Kasumi also died holding off the Collectors.

Ashley Williams (I chose Kaidan on Virmire)
Mordin Solus (Died curing the Genophage)
Thane Krios (Mortally wounded saving the Salarian Councilor)
Legion (Died uploading the Reaper Code to all Geth)
Zaeed Massani (Failed to gain his loyalty in ME2 and he died saving the Volus Ambassador.)
Miranda Lawson (sided with Jack during their confrontation ME2 and she died after biotic throwing her dad out the window.)
The Renegade resolution to Conrad's quest in ME2 and Shepard's anger towards their clone over their hamster.
