[[Cerberus]] helped liberate him from an Alliance prison, and [[The Illusive Man]] used Leng as his best wet-work assassin. Leng was charged with hunting down and capturing [[Paul Grayson]] when his identity was confirmed on [[Omega]]. Kai and a squad of six other Cerberus operatives captured Grayson and killed [[Liselle]], the daughter of [[Aria T'Loak]]. Kai Leng was ordered to oversee Grayson during the [[Reaper]] nanotechnology experiment and exterminate him when he got out of control. When [[turians]] raided the research facility that harbored Grayson, Kai chose to rescue The Illusive Man rather then kill Grayson. After the assault, The Illusive Man sent Kai as a representative of Cerberus to strike a deal and keep an eye on Aria.

After Grayson escapes Omega, Kai Leng offers Anderson and Sanders a ride off of Omega, in exchange for their help in taking out the Reaper-possessed monster he had a hand in creating. However, he is met only with refusal, for neither wanted to help Cerberus. They both attack him, but prove no match for the highly trained assassin. Instead of killing them both, he offers them a second chance, since The Illusive Man had stated before that Sanders was the key to finding Grayson. They relunctantly accept; however, they later betray him when confronted with turian security guards near Leng's shuttle. With a distraction Anderson tasered him, then convinced the guards to let him take Leng's limp body. Leng was then tied to the copilot chair with rope from the shuttle's med kit.

His prowess in combat and xenophobic nature drew the attention of the [[Illusive Man]], and [[Cerberus]] helped liberate Leng from prison. The Illusive Man used Leng as his best wet-work operative and assassin for over a decade, realising he was not only ruthless but discreet and methodical.
Upon arrival at Grissom Academy, the facility used for the Ascension project, both Anderson and Sanders depart the shuttle, leaving him tightly bound to the chair. He manages to free himself after an intense struggle with the ropes that bound him; however, it caused his limbs a great deal of pain as they were bleeding afterwards. He later confronts a gifted biotic student named Nick, and manages to persuade him that he is on his side. He hears Grayson in the distance, prompting both of them to hide in a nearby empty room. He formulates a plan to ambush Grayson using Nick's biotics. However, the assassin was unarmed and therefore had to attempt to steal Grayson's weapon. He ultimately failed to obtain his weapon, only to be saved by Anderson who fired his shotgun twice at the Reaper-controlled Grayson, killing him. Anderson refused to simply let Leng go; however, Nick had been shot and needed medical attention, giving Leng the leverage he needed to escape. Anderson chose to let Leng go, after cruelly shooting him in each leg. Despite his limp legs and injuries, Leng still manages to get back to the shuttle and escape.

After the assault, the Illusive Man sent Leng to Aria as Cerberus' representative, both to negotiate for her assistance in finding Grayson and to keep an eye on her.
